Physician Order & Prior Authorization — Week 1–2

Our coordinators contact your loved one's primary care physician (or specialist) to obtain a physician's order for home health aide services. We prepare and submit the Medicaid Prior Authorization (PA) request with comprehensive clinical documentation. PA approval typically takes 5–10 business days. We follow up and handle any appeals if the initial authorization is insufficient.

HHA/CNA Training Enrollment — Week 1–3

Complete 80–85% of coursework online through our learning platform, on your own schedule. Competency Evaluation & Certification — Week 4–6

Pass the final competency evaluation (written + skills demonstration with RN evaluator) and receive your state-recognized HHA or CNA certification. Family HHA submits your certification to the state registry and begins your direct employment onboarding.

Agency Placement & First Paycheck — Week 5–8

Family HHA hires you directly as a home health aide to provide care for your family member under the Medicaid-authorized care plan. Complete the agency onboarding paperwork, begin providing authorized care hours, and receive your first paycheck — typically within 1–2 pay cycles of starting.

What Happens After You're Certified?

PA Renewals: Medicaid Prior Authorization typically renews every 60–90 days. Family HHA coordinates all renewal paperwork for you — ongoing, at no cost.
Supervision: As a Medicaid HHA, you have monthly supervisory visits from a registered nurse who reviews the care plan and your documentation.

For your loved one to receive Medicaid home health aide services — and for you to be paid for providing them — they generally need to meet these criteria:

Active Medicaid enrollment
Your loved one must be enrolled in traditional (fee-for-service) Medicaid. Not Medicaid Advantage. Family HHA helps check Medicaid status and assists with enrollment if needed.
Need for assistance with activities of daily living (ADLs)
Medicaid must document that your loved one needs help with bathing, dressing, eating, mobility, or toileting due to a medical condition. This is almost always met for the conditions we work with.
A physician's order
A primary care physician or specialist must order home health aide services. Family HHA coordinates with your loved one's doctor to ensure the order reflects their full scope of need.
Community living (not in a nursing facility)
Your loved one must live at home or in a community setting — not in a skilled nursing facility. Home, adult family home, and assisted living can all qualify in most states.

Can a family member really be a paid HHA for their loved one?

Yes — this is the foundation of Family HHA's program. In most states, a family member who completes state-approved HHA training and certification can be hired directly by Family HHA to provide paid home health aide services for their loved one, with Medicaid covering the cost through Prior Authorization (PA). The family caregiver receives a W-2 and hourly wages just like any professional HHA.

What is Prior Authorization (PA) in Medicaid home health?

Prior Authorization is the Medicaid approval process that must happen before paid home health aide services can begin. A physician writes an order for home health services, and Family HHA submits that order with clinical documentation to Medicaid for approval. How long does HHA training take?

Most Family HHA students complete the full training program in 4–8 weeks. The online portion (80% of the curriculum) is self-paced — how quickly you progress depends on how many hours per week you can dedicate to the modules. The in-person skills demonstration is typically scheduled in 1–2 sessions. There are no minimum weekly hour requirements — you work at your own pace.

Does my loved one need to have a specific condition?

Not a specific diagnosis, but they do need a documented medical need for home health aide services — meaning assistance with activities of daily living (ADLs) due to illness, disability, or functional limitation. Common qualifying conditions include Alzheimer's disease, Parkinson's, stroke effects, COPD, heart failure, spinal cord injury, developmental disabilities, traumatic brain injury, and many others. Can a spouse be a paid family HHA?

Will I still get paid if Medicaid changes my loved one's authorized hours?

The PA authorization determines your paid hours — if authorized hours change, your paid hours change. This is why Family HHA's ongoing care coordination matters: we work proactively to ensure that your loved one's full care needs are documented and reflected in the PA, and we handle all renewal paperwork. If hours are reduced inappropriately, we assist with appeals.

Can I be a paid family HHA AND receive pay from a Medicaid Waiver program?
